Commissioners accused her of a "muddled view of the law" after it emerged she authorised investigations into eight senior crime intelligence officers without a proper legal trigger, relying only on Fadiel Adams' sworn statement about classified documents pushed under his door — documents she never saw. Justice Mjabuli-Madlanga pressed her on why no lawyer in her office flagged the missing statutory basis, while she admitted giving Parliament incorrect information in November. She also blamed a subordinate for the blunder.
