Godfrey LeBea told the Madlanga Commission that private investigator Paul O’Sullivan sought an update on the Phala Phala investigation while with former presidential adviser Dr Bajani Chauke; LeBea refused.

LeBea said Crime Intelligence was achieving only 15% of required vetting, describing the system as failing and saying an intervention had not helped because senior generals were not on good terms.
