Justice Madlanga said the Commission may recommend dissolving IDAC after evidence of governance failures, ignored procedures, unlawful investigations and internal instability.

Head of investigations Matthew Sisoko said IDAC’s operations manual was formally signed on 16 October 2024 and shared with staff, contradicting claims that it was only a draft.

Sisoko also said documents not in the original record were inserted into an active case file, and chief investigator Dylan Piramal feared arrest after the discovery.
